Working with the resident’s physician, our staff – including medical specialists, nurses, nutritionists, dietitians and social workers – establishes a comprehensive treatment plan intended to restore you or your loved one to the highest practicable potential. Courtland Gardens Health Center accommodates 162 residents. Courtland Gardens has been serving the greater Stamford community and Fairfield County for over 40 years striving to provide quality care, in a comfortable, friendly atmosphere.
Courtland Gardens has three care units including short-term rehabilitation, dementia specialty care, and long-term care. We provide an environment that nurtures life, celebrates choice and encourages autonomy, and where the human spirit flourishes no matter the resident’s age or health situation.
At Courtland Gardens, our goal is to provide compassionate care. We strive to help each person we serve improve his or her level of functioning and have a better quality of life. At the time of admission, individual assessments are completed resulting in treatment plans that are outcomes focused, and responsive to physical, social, emotional and spiritual needs. Each plan reflects the collaborative efforts of the resident, family and the professionals on our multidisciplinary care team.
